Healthcare Roundtable-UMass Memorial's Hospital at Home

UMass Memorial has announced the offering of their new Hospital at Home program.  Hospital at Home is an innovative way for patients to receive high-quality, hospital-level care in the comfort and familiarity of their own home. With UMass' outstanding clinicians visiting their homes several times a day and telehealth technology keeping them connected to their health care team, they bring the hospital to their home.

Constantinos (Taki) Michaelidis, MD, MBA, MS
Constantinos (Taki) Michaelidis, MD, MBA, MS is an internal medicine physician by clinical training and practices as a virtual home hospitalist in the UMass Memorial Health (UMMH) Hospital at Home program where he is medical director and faculty in the Program in Digital Medicine at the University of Massachusetts Medical School. Prior to joining UMMH, he was senior medical director in the Atrius Health home hospital program. There, he supported local and national home hospital program development and developed and scaled new acute and post-acute home-based care models for a predominantly frail, medically complex and home-bound population in eastern Massachusetts.
